Today one senior told me an interesting and satisfying case. Kind of funny in a way if you look at the history. The pain just run here and there everywhere all over the body.
One teenage guy came in to hospital complaining of joint pains.
Case History:
According to the patient, this is what happened:
Had sprained his right ankle 2 weeks ago during a soccer game and went to a traditional healer and recovered fully. No more pain.
Following that he said joints on his LEFT start to feel pain. Pain in his LEFT ankle, knee, hip. As the week past by, the pain lessen. But funny thing happened, the upper Left limbs, his arms, wrist start to feel pain.
Ok...that means first pain in the right ankle due to accident but following that he was presented with joint pains in other unaffected areas.
On physical examination, other than the reduced responses due to pain in joints and fever he is perfectly well. No injuries, no bruises, nothing. His heart rate is slightly raised but not yet tachycardia. But upon auscultation, there is a faint pansystolic murmur.
My senior asked the guy did he have had a sore throat recently. After thinking for like an hour, he finally replied, Yes.
Tests were done and results confirmed. The teenage guy has rheumatic fever. So happy ending for everyone. That sprained ankle is like a extra 'bonus' for that boy.
